To allow us to run the VBS program at Hillside, we are required to screen the campers daily. To make this process more efficient, we are asking parents or guardians to fill out the following form daily on the morning of each day of camp. Please fill out a separate form for each camper you are responsible for. This will save being in long lineups while we screen each child in person. Your assistance with this requirement will allow us to make camp a safer and more enjoyable experience for your children.
